Dear Dr. Murley:

Millstone Nuclear Power Station, Unit No. 3 Reporting of Potential Significant Deficiencies in Design and Construction Tubeco Weld Radiographs (SD-56)

In a May 19, 1984 telephone conversation between your Mr. T. Rebelowski and our Mr. R. R. Viviano, Northeast Nuclear Energy Ccmpany (NNECO) reported a potential significant deficiency in the construction of Millstone Unit No. 3 as required by 10 CFR 50.55(e). The potential significant deficiency involves vendor (Tubeco) supplied radiographs. A random sample of 125 out of a population size of 2,740 welds were reviewed. A total of 54 welds were found to be out of vendor procedure and ASME 111 requirements due to film density violations, the placing of penetrameters and penetrameter identification numbers in the weld, undocumented linear indications, and excessive geometric unsharpness.

The subject reported deficiency resulted from a different review of Tubeco radiographs than that which was previously performed to address NRC violations identified in IE Inspection Report No. 50-423/83-14 (Reference 1). The first two of these concerns (film density violations and the placing of penetrameters in the weld), were the subject of IE Inspection Report No. 50-423/83-14. In both cases, these occurrences were not determined to be technical problems or safety concerns (Reference 3). However, it is the three remaining concerns identified in this review of Tubeco radiographs (undocumented linear indications, excessive geometric unsharpness, and penetrameter identification numbers in the weld) which lead us to report this potential significant deficiency.
