Radwaste (00E) Outfall Code No: 800217 Boronj otal Chromium Parameter Code No: 01022, 01034 T Parameter: 1. Dste of Incident: March 11 to March 14, 1984 2. Identification of specific effluent limitation or monitoring requirement affected: The NPDES Permit requires radwaste releases be analyzed for boron and total i chromium content. 3. Description of Incident: From March 11 to March.14, 1984, 87,500 gallons of water from the condensate storage tank was released without being monitored for boron and total chromium. This oversight was discovered on March 15, 1984 at 1500 hours. 4. Apparent Cause: Plant Procedure HP3.6 did not identify boron and chromium as required analyses of the condensate storage tank if the tank was released as a radwaste batch. 5. If not immediately corrected, the anticipated time the condition is ~ expected to continue: NA. 6. Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence: Plant Procedure HP3.6 will be revised to include the required monitoring. - Parameter: 1 Total Residual Chlorine (TRC)
Parameter Code No: 50060 - 1. Date of Incident: 2/17/84 2.- Identification of specific effluent limitation or monitoring requirement affected: The NPDES permit requires three grab samples of the plant discharge to be collected and analyzed for TRC during the chlorination of the plant circulating and cooling water systems. 3. Description of Incident: On 2/17/84 during chlorination.of the service water system,' only two grab samples for TRC were collected instead of the required three samples.
- 4. Apparent Cause:
Technician error. The technician had not been involved with chlorination procedures for the past six months. The plant's chemistry procedures 'specifically require that three grab samples for TRC be collected during chlorination. 5. If not immediately. corrected, the anticipated time the condition is expected to continue: NA
- 6. ' Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:
All technicians were reminded of the TRC sampling requirements as well as other responsibilities and requirements of the NPDES permit. The plant's chlorination data sheet is also being revised to include the NPDES sampling requirements during a chlorination. 7. ~ Additional Comments: The second sample of the plant discharge was taken at the end of chlorination and measured less than detectable (<.02 ppe) TRC. 8. Description:
Intake (000) and Discharge Outfall Code No: 800149 to Lake Michigan (001) 800095 Parameter: Temperature and pH Parameter Code No: 00011 00400 1. Date of Incident: 1/7/84 and 2/4/84 2. Identification of specific effluent limitation or monitoring requirement affected: Continuous monitoring of intake and discharge temperature and pH is required. 3. Description of Incident: On 1/7/84 the pH recorder, pHR 1312, was found to be malfunctioning. The pH readings were erratic, fluctuating off scale from low to high. The pH recorder remained out of service until 2/3/84. On 2/4/84 the temperature recorder, TR 5370, failed to operate properly. The recorder was out of service until 2/8/84. 4. Apparent Cause: ~ The ribbon cable on the pH recorder shorted out to the instrument case. The temperature recorder had a faulty switch and ribbon cable.
- 5. -If not immediately corrected, the anticipated time the condition is expected to continue:
NA 6. Corrective Action - The'fo11owing corrective action was taken or will be taken to reduce, eliminate and prevent recurrence: The pH recorder was removed from service. A maintenance order was filed. The ribbon cable was replaced and a loose wire to the probe was also repaired. The pH recorder was calibrated and returned to service after repairs were completed. The recorder ribbon cable will be made a stock item. A maintenance order for the temperature recorder was initiated. The recorder's switch was repaired and the ribbon was replaced. The temperature reco.ier was then returned to service. 7. Additional Comments: While the pH recorder was out of service, the discharge to Lake Michigan was grab' sampled and analyzed three times per day for pH. The plant is investigating placement of a redundant temperature recorder for each parameter to be placed at a local sample point to minimize recurrence of this event. t 8. The lost oil was trapped in the snow around the vent pipe and the south side of the tank. i c ...a. c Clean-up was immediate. The effected snov was removed and placed in 8 barrels. Oil dry vill be added to the be.rrels and they will be disposed of according to regulations in a 1 licensed disposal facility. Identification of specific effluent limitation or monitoring requirement affected: The Turbine Sump 011 Separator Wastewater is required to be sampled and analyzed for oil and grease once per week. 3. Description of Incident: During the week of November 20 through November 26, 1983, the Turbine Sump Wastewater was not sampled. 4.. -Apparent Cause: Failure to take routine sample on 11-21-83. 5. If not immediately corrected, the anticipated time the condition is expected -to continue: NA 6. Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence: A sample was collected on 12-1-83 when the oversight in sampling was discovered. This matter was also discussed at the weekly Technician meeting. A weekly checklist of required sampling and analyses was prepared. Additionally, a Plant Procedure CH 9.1, Site Surveys, is under development. 7. Additional Ccmments: Daily visual observations of the final discharge to the lake showed no oil or grease during this period.
