Dear Sir:

This special report is submitted in accordance with LaSalle Unit #2 Operating license NPF-18, Appendix A Technical Specification 3.3.7.12 and 6.6.c.

On May 4, 1984, the loose parts monitor channel #9 was inoperable for greater than 30 days per Technical Specification 3.3.7.12.a/b.

The degraded status results from an open connection in the micro-dot cable which runs from the pre-amplifier to an accelerometer. This is caused from construction activities at the 820 level within the drywell. Maintenance, during a short outage pulled a new micro-dot cable to the accelerometer.

During the connection of the newly pulled cable and the accelerometer, a small female to female union was accidently dropped and lost. 'the station storeroom and instrument maintenance shop could not locate a spare, so the union (with spares) was ordered 1sunediately. Channel #10 is 180 degrees apart and provides redundant coverage of the area of the vessel that was being monitored by Channel #9.

' At the next opportunity after the part is available and the Unit is in cold shutdown the union will be installed. AIR-01-84-67082 was written to track this work.

This report is submitted late due to an oversight by the personnel responsible for reporting the inoper.sble equipment. These personnel will be reminded of the importance of prompt reporting of inoperable equipment required by the Technical Specifications, n
