Dear'Dr. Bettenhausen:
During-the week of Lecember 2,-1991, Licensed Operator Requalification-Examinations were: administered to eight; Millstone' Unit 3-Licensed-Operators'and-Senior--Licensed Operators._'These examinations were conducted in accordance with NUREG 1021, Operator Licensing-Examiner Standards, section ES-601.
Accordingly, the examinations were prepared, administered, and evaluated by both NRC and' facility examiners.
Preliminary: results of-the facility evaluations for all portions of
.the examination were provided to K.
Ihnen, NRC Cheif' Examiner, on December 6, 1991.--Attached-is.a summary of our grrdes.
LAn' evaluation.of the examination results-was-_ performed to-identify strenghts.~and weaknesse_s,_both individual and crew,'and to identify necessary remediation and enhancements tolthe MP3 Licensed Operator-Requalification' Program content.. General-Throughout all portions of this exam, there were several instances of procedureal step omission.
Most of these were either inconsequential or later corrected.
Exceptions on an individual basis are noted below.
Operator C-During the demonstration portion of this examination, this operator was slow in reducing ArW flow to a ruptured steam generator.
As~ shift supervisor, extra on-shift personnel were not summoned to complete incident report forms, as required by unit management.
During performance of a JPM, this operator omitted a procedural step, resulting in unsat task performance.
-Operator B-Both as Supervising Control Operator and in JPM performance, this operator was unusually slow in the performance of tasks or directing task performance.
Operator H-As Reactor Operator during the demonstration portion of the exam, this operator omitted a procedural direction to provide power to motor-operated valves.
This omission was corrected by the crew.
Operator G-During performance of a JPM, this operator omitted a procedural-step,.resulting in unsat task' performance.
Operator E-During performance or a JPM, this operator omitted a procedural step, resulting in asat task-performanae.

.Page-3 of.3 WALKTliROUGH EXAMINATIONS There were a total of 3 JPM's performed unsatisfactorily, with one JPM performed unsatisfactorily by more then one examinee.
The JPM was -
"JPM-59, rill-a safety Injection Accumulator"..D1ficiencies noted during the performance of this JPM will be addressed in accordance with the Licensed Operator Re,ualification Training Program.
WRITTEN EXAMINATION Examinee performance on the written examiration was satisfactory, showing an understanding of the learning objectives examined.
