Notification of Problen/ Defect H. B. Robinson Steam Electric Plant (NRC Report per 10CFR Part 21)

Dear Mr. Koppler:

Baxter Fluid Power (Anker-Ilolth) was informed by telephone on May 25, 1984 by a Mr. Rick Dayton of Carolina Power and Light (H. B. Robinson Plant), of a potential problem / defect concerning static load rating on Anker-Holth/McDowell Wellman Engineering Part Nunber 21.12620.008 serial number 001.

Part Number 21.12620.008-001 is one (1) of twelve (12) supplied to EBASCO Services for the H. B. Robinson plant in 1969.

The above part is a hydraulic suppressor (snubber) assembly with a tension and compression load rating of 470,000 pounds (470 Kip).

According of information received from Carolina Power and Light, during a static load test one of the self aligning ball bushings (Torrington part number 30SBB48) failed below the 470 Kip rating at 412,000 pound (412 Kip).

At time of hydraulic suppressor (snubber) manufacture in 1969, the static radial load rating of the Torrington bearing (30SDB48) was 899,000 pounds (899 Kips) and one (1) of the twelve (12) suppressors was tested to, and met the 470,000 pound (470 alp) beading specifications.

Subsequent to manufacture, the Torrington bearing (30SDB48) rating has been revised downward to 300,000 pound (300 Kip) static radial load rating.

The safety hazard created by a failure of the self aligning ball bushing is the potential non-functioning of the appropriate suppressor due to critical nin-alignment from failure.

An additional location with hydraulic suppressors (snubbers) using the Torrington 30SBB48 self aligning ball bushing is Wisconsin Electric Power Conpany, at the Point Beach nuclear plant. A quantity of twelve (12) suppressors of a 500,000 pound (500 Kip) rating are installed at. the above location with an original manufacture date of 1969. Baxter Fluid Power, Anker-IIolth, formerly Anker-liolth/McDowell Uc11 man Engineering, has withdrawn from the design of new suppressor systems, and only supplies replacement parts for designs of previous manufacture.

As of May 31, 1984 Baxter Fluid Power, Anker-liolth completed the formal conveyance of all design rights, documents, and data to Paul-Munroe flydraulics, Inc.,1701 West Sequoia, Orange California 92668 (Contact tir. John !!cCabe) 714-978-9600.

Recommend corrective action is to replace the existing Torrington self-aligning ball bushing (30SBB48) of the now existing 300,000 pound static radial load rating, with a new self aligning ball bushing with a static radial load rating exceeding the load rating of the suppressor, using Paul-Munroe !!ydraulics part number PA-89467.

The noted part number PA-89467 is directly and dimensionally interchangeable with the existing bearings.

The length of time to complete the above noted recommended corrective action is estimated at up to one year (twelve months) allowing for sufficient time to procure the replacement parts, schedule the suppressor facility of outage, renovo the suppressors from their mountings, de-contaminate as required, re-place the bearings, and re-install.

We are currently reviewing our other previous designs and will issue a subsequent report as appropriate per 10 CPR part 21.
