Millstone Nuclear Power Station, Unit No. 3 Reporting of Potential Significant Deficiencies in Design and Construction:

Isolator Cabinets, Main Ventilation, Air Conditioning and Other Miscellaneous Panels with Improperly Crimped Lugs (SD-53)

In a February 15, 1984 telephone conversation between your Mr. T. Rebelowski and our Mr. R. E. Lefebvre, Northeast Nuclear Energy Company (NNECO) reported a potential significant deficiency in the construction of Millstone Unit No. 3 as required by 10CFR50.55(e). The potential significant deficiency involves inadequate lug crimping in isolator cabinets, main ventilation, air conditioning, and other miscellaneous panels supplied by Systems Control.

Our architect-engineer, Stone & Webster Engineering Corporation, has completed a one-hundred percent lug crimping inspection for all pre-turned-over panels supplied by Systems Control. Repair work for these panels will begin in mid-June, 1984. NNECO has completed a similar inspection program for all post-turned-over panels supplied by Systems Control and approximately seventy-eight percent of the repair work has been completed for these post-turned-over panels.

The inspection programs revealed that approximately forty to sixty percent of the terminations in the panels supplied by Systems Control for Millstone Unit No.

3 had incomplete or incorrectly crimped lugs tha't met the rejection criteria specified in the manufacturer's Instruction / Maintenance / Inspection Manual. The repaired lugs will be installed to meet the manuf acturer's installation criteria.
