Dear Mr. Keppler:
INSPECTION REPORT NO. 50-483/78-04 This is in response to your letter of June 22, 1978 reporting results of an inspection at Union Electric Company's Callaway Plant Site on April 24-25, 1978 and as detailed in the inspection report number 50-483/78-04.
None of the material in the inspection report or in this response are considered proprietary by Union Electric Company.
The response listed below corresponds to the item listed in Appendix A, Notice of Violation of the inspection report.
Deficiency Contrary to 10CFR50, Appendix B, Criteria XV, and the Callaway PSAR, Section 17.1-15 and procedure WP-ll5, an embedment was repaiced ut,ing heat without proper procedural control of the-heat applied.

Corrective action taken and the results achieved:
NCR 2-2736-C-A was written describing i
the nonconforming condition which was subsequently dispositioned incorrectly.
NCR 2-3419-C-A was written redescribing the nonconformance with a recommended disposition "use as is".

Keppler July 25, 1978 a form for concrete at the top of a stairway, is not a load carrying. member and is not structurally significant.
A thorough visual inspection of the plate was conducted and no cracks or detrimental effects from the repair work were found.
The Archit';ct/
Engineer, in his response to the NCR, concurred that the plate was acceptable for "use as is".

Corrective action to be taken to avoid further noncompliance:
The Ironworker General Foreman and fore-man in the control building have been informed of this procedure violation and instructed to monitor work more closely to ensure that proper control of applied heat is maintained when conducting plate repairs of this type.
Additionally, those responsible for in-correct dispositioning of the original NCR will be reinstructed regarding actions necessary to close an NCR.
