MPQAD Procedure N-12, Rev. 3 O HOLD POINT NOTIFICATION CONDITION DETAILS MPQAD Procedure N-12 (Rev.
3, Page 4, Para. 5.4.1) states in part:
Document an inaccessible item /all inaccessible inpsection activities (attributes) on a
Disposition for an Inaccessible Item / Attribute (s) (DIIA) Form.
Contrary to the above, PQCI E-5.0 (Rev.
15, Page 9, Special Instruction No. 6) states in part that the requirements of Activities 3.2, 3.3 and 3.5 shall be implemented on the visible portion of the cable and noted on the IR in block 15, the portion of the cable not inspected and the reason it is not inspected.
For VIRs:
V344, V346, V347, V348, V349, V350, V352, V353 & V2822,* when MPQAD complied with PQCI E-5.0, they did not initiate the required DIIAs.
CPCo is requested to provide clarification of the intent Procedure N-12 with regard to cable reinspections.
