P-1.90 QVP - Module 800 0 HOLD POINT NOTIFICATION CONDITION DETAILS l

Upon completing verification of QVP activities in Module 800 E, F,

and G, the following was discovered in addition to Item Number 081.

a.

QCIR VP-1.90-618-1-38:

Block 17 is not signed.

b.

Field Weld FW-59 is not identified on the lug attached to OHBC-34, Drawing M-618, Sheet 1(Q), Rev 17/F1, or hanger SK 618-1-509, Rev 0/B1. Therefore, QCIR VP-1.90M-618-1-59 d is not traceable to a particular lug.

c.

A DIIA fann was completed on the pipe associated with FW-38, QCIR VP-1.90 M-618-1-38 1

because there are wooden wedges driven in between the pipe and hanger 618-1-6 to move the pipe away from the hanger.

Is the use of wooden wedges to achieve location in piping systems a documented, accepted practice?

c-2)

Is there any evidence available that the stress caused by the use of these wedges has been evaluated?

c-3) Under what circumstances was this support accepted by MPQAD?

c-4) At what point in time was this wedge added?

Specifically, before or after fit up?

c-5) Since this is obviously not a pemanent condition, why couldn't these wedges be removed for QVP?

Please advise CIO of your resolutions to these conditions.
