-FINAL REPORT OF SIGNIFICANT CONSTRUCTION DEFICIENCY NO. 102
" FAILURE TO INSTALL DOWELS AND SHEAR BLOCKS" INTRODUCTION This report is_ submitted pursuant to 10CFR50.55(e).
It describes the problem of not installing dowels and/or shear blocks on the emergency feedwater pumps,
- motors, turbine, and the emergency diesel generators.
To the best of our knowledge, this deficiency has not been reported to the USNRC pursuant to 10CFR21.
DESCRIPTION Through an internal design review around February 7, 1984, it was discovered that the dowels and/or shear blocks required by vendor's seismic analysis had not yet been installed on the emergency feedwater pumps, motors, turbine, and the emergency diesel-generators.
SAFETY EVALUATION These dowels and/or shear blocks are required by the vendor's seismic analysis, which is available in the Seismic Qualification Review files. We believe that this' event is reportable because there is no assurance that this equipment would have performed its required safety function following a design basis earthquake without the installation of these dowels and/or shear blocks.
CORRECTIVE ACTION
.A review of safety related pumps and rotating machinery was conducted pertaining to the vendor's seismic analysis. The only exceptions to these analyses is on the emergency diesel generator and emergency feedwater pumps, motors, and turbines.
Installation of the dowels and shear blocks for the emergency diesel generators was completed per Condition Identification and Work Authorization ~(CIWA) No. 840443 (replaced 840838) and 841901.
Installation of the dowels and shear blocks for the emergency feedwater pumps, motors, and turbines was completed per CIWA 840839.
Engineering and Quality
- Assurance has reviewed all the supporting documentation.
This report is submitted as the Final Report.
