'" DIESEL GENERATOR."A" FUEL OIL PUMP" INTRODUCTION This report is submittal pursuant to 10CFR50.55(e), the report describes the
' failure of a fuel oil pump which subsequently caused a shutdown of the 3A-S Diesel Generator.
This deficiency is not considered reportable pursuant to 10CFR21.
DESCRIPTION On April 6, 1983, while troubleshooting governor problems associated with Emergency Diesel Generator (EDG) 3A-S, the engine fuel oil pump bearing failed. The failure was sudden and was witnessed by personnel monitoring the EDG. The cause of the failure has.been investigated but could not be ascertained. The deficiency was initially evaluated as not reportable.
During a Joint Test Group review of the "EDG" Reliability Report" the deficiency was reevaluated as reportable.
-SAFETY IMPLICATIONS Had the deficiency gone uncorrected the failure of the fuel oil pump could have prevented the "A" EDG from carrying out its design function during a loss of offsite power.-
!J CORRECTIVE ACTION The fuel oil pump was immediately replaced with a spare and tested satisfactorily. No other fuel oil pump failures on either engine have occurred since the time of this occurrence.
The vendor was contacted to determine if other failures had occurred. The vendor indicated that no other failures of KSV fuel oil pumps have been identified on nuclear grade equipment and the occurrence is isolated to

W To ensure that deficiencies are properly evaluated when first identified, the Nuclear operations QA Group has conducted training and upgraded the approved program for deficiency evaluation.
