Dear Sir:

This lette. provides information required by Millstone Unit 2 Technical Specification 3.3.3.1.b, Table 3.3-6, Action 17 and constitutes a Special Report required by that specifica ion.

At 1430 on July 14,1991, the Unit 2 stack high rance radiation monitor RM 8168 was declared inoperable. Intermittent failures caused by fluctuations in the output of its power supply had been observed to cause the monitor to automatically halt its operation.

These interruptions occurred at a frequency of approximately once per day. This monitor is microprocessor based and is designed to detect fluctuations in its power supply's performance and save its programming information prior to a total loss of power supply output.

Guidance had been provided to operations personnel on how to reset the monitor to operating status. This allows the monitor to be immediately restored to service during the period that preparations are being made for the repair of the spare power supply.

Th: necessary repair parts were ordered on July 16 and iepairs are currentl underway to 3

rebuild and install a spare power supply with an adequately stab!c output to maintain long term continuous operation. These repairs and subsequent testing are expected to be completed by July 31, 1991.
