TVA letter to NRC dated July 12, 1989, "Sequoyah Nuclear Plant (SQN) - Unit 2 Cycle 3, 90-Day In-Service Inspection (ISI) Report" Er. closed is SQN's Unit 1 Cycle 5 ISI Summary Report (Volumes 1 ar.d 2)-
that contains a hintorical record of repairs, replacements, modifications, and in-service nondestructive examinations that were performed on American Society of Mechanical Engineers (ASME) Code Class 1 and 2 components from October 4 to December 14, 1991.
This ISI Summary Report is being provided in accordance with IWA-6230 of ASME Code,Section XI.
In addition to the subject report, TVA is providing supplemental' information to SQN's Unit 2 Cycle 3, 90-Day ISI Report (previously provided by the referenced letter) as was discussed with NRC. The supplemental information contains the reports required by the ASME code and associated with a modification to_SQN's essential raw cooling water system-(replacement of a carbon steel section of piping and valves with stainless steel). This was identified as a reporting discrepancy during a final review of Workplan 11850.- This oversight is believed to be i
isolated; however, a more rigorous system has been established for ensuring the proper closeout of ASME,Section XI, work packages, which should prevent recurrence.
