Final Report of 10CFR50.55(e) Item on Agastat Relay Bases (#23)
Dear Mr. Keppler:
This is Detroit Edison's final report involving Agastat relay bases.
This item was originally reported to Mr. R. Knop of NRC Region III by Detroit Edison's Mr.11. A. Walker, Supervisor - Construction Qu- 'ty Assurance, on November 9,1979.
In performing checks on the Primary Containment Monitoring System, some circuits through the Agastat relays remained open when they should have been closed.
Investigation revealed that the female pins in the relay mounting bases were loose, and if a good alignment of the relay pins (male) to the mounting base sockets (female) was not ob-tained, the female sockets were pushed away from the relay resulting in an open or intermittently open circuit. When the 10CFR50.55(e) was initiated, the problem was noted in two panels of the Containment Monitoring Systems (panels H11-P914 and H11-P915).
Subsequent investigation revealed this type of relay base was also used in thirteen (13) other panels supplied by General Electric.
Field Modification Requests (FMR) 1466, 3756, 4111, 4112, 4113, 4114 and 4115 were written by Edison Engineering to replace the defective relay bases (type CR 0095) with the new (type ECR 0002) in the af-fected General Electric supplied cabinets.
The replacement has taken place, and the work has been inspected to assure compliance with the-above FMRs.
