Gentlemen:
On November 11, 1991. TV Electric identified a lack of weld penetration on the lef t bank starting air monif old assembly for Linit E. Trdin
'B' (mergency Diesel Generator.
Our last report, logged TXX 92028 and dated January 10, 1992, stated that the reportability evaluation was scheduled for completion on April 13 1992, Examination of the weld section revealed that the amount of weld reinf orcement more than made up for t he lack of penet ration such that the weld thic} ness equals or exceeds the adjacent pipe wall thickness.
The section of pipe was hydrostatically tested at 1.5 times the design pressure without any leakage.
Additionally, the stress report stated that the welds were not assumed to have complete penetration in the root area and were discounted 1/16 inch.
Therefore, no substantial safety hazard exists and this condition is not reportable pursuant to 10CFR50.55(e) and 10CfR21.
The installation was repaired using site procedures by adding a new elbow and pup piece.
