J. M. Farley Nuclear Plant NRC Inspection of Jarmary 11 - February 5,1984 RE:
Report Numbers 50-348/84-03 50-364/84-03
Dear Mr. Verre111:
This letter refers to the violation cited in the subject inspection reports which states:
"The following violations were identified during an inspection conducted on January 11 - February 5,1984. The Severity Levels were assigned in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C).
Technical Specification 6.8.1 requires that writtten procedures shall be implemented for the control of maintenance activities.
Contrary to the above, the licensee did not implement procedures for the control of maintenance activities. FNP-0-AP-16 (sic) requires' that a clear rid concise description of the cause of a problem be documented on a maintenance work request. The-licensee disasse:nbled an RHR suction relief valve and discovered that the disc ring pins were missing. No entry was made on a maintenance work request concerning the missing rings."

Mr. D. M. Verre111 March 8, 1984 Page Two Reason for Violation This violation was caused by personnel orror in that the personnel involved failed to follow approved procedures.
Corrective Action Take_n and Results Achieved The personnel involve 6 have been counseled concerning this incident.
In addition, all maintenance personnel will be retrained on the requirements of FNP-0-AP-15 (Maintenance Conduct of Operations) with emphasis on the proper document 9 tion of as-found conditions and probable causes of equipment failures. The as-found condition of valve Q2E11V015A will be added to the plant documentation system.
Corrective Steps Taken to Avoid Further Violations See above. All corrective action is scheduled to be completed oy March 15, 1984.
