NRC Letter to Boston Edison, dated February 9,1984
Dear Mr. Starostecki:
This letter is in response to the violation identified during a routine safety inspection conducted by Mr. J. Johnson of your of f ice on November 8 to December 31, 1983, and communicated to Boston Edison Company in Ap-endix A of the reference.
Notice of Violation (VIO 83-24-01)
Technical Specification 4.6.A.I requires that during heatups and cooldowns, the following temperatures be permanently logged at least every 15 minutes:
a) Reactor _ vessel shell adjacent to shell flange, b) Reactor vessel shell flange and, c) Recirculation loops A and B.
j Contrary to the above, on December 11, 1983, between ll:00 a.m. and 4:00 p.m., a cooldown of the reactor vessel was performed (via the head spray mode of the Residual Heat Removal System).from 390*F to 150'F without I
logging the required temperatures every 15 minutes.

Response
Our'immediate corrective action was to review all appropriate charts to l
ensure that the cooldown rato did not exceed 100*F/ hour. This was success-l fully verified since Charts TR-263-105 on Panel 904 (Rx Vessel Temp 2raiure Shell and Flange) and TR-151-A&B (Recirculation Suction Temperature) indi-cated that during the event in question, the cooldown rate never exceeded 60'F/ hour.
To preclude recurrence, an instructional memo was issued on 2/24/84, to all Watch Engineers and Supervisors to reemphasize the importance to ad-here to procedures and will require them to review Station Procedure 2.l.7-(" Vessel Depressurization and Cooldown").
