identifies the activities that have been completed and/or are in progress, to resolve the issue of nonconforming material supplied by the Tube-Line Corporation (T-L).
An investigation has been performed by Bechtei San Francisco Central ' upplier S
Quality (SFHO) Department to determine if any sub-suppliers have supplied Tube-Line material to the Midland Plant. The following is a. summary of this investigation and of the corrective actions taken:
1.
To date, of the 413 letters nailed from SFHO, 65 letters went to vendors that supplied material to the Midland project. Bechtel Ann Arbor has received 84 (99%) of the Midland responses.
2.
Of the 84 responses, 81 vendort have responded that no Tube-Line material was furnished to Midland. The three vendors that have furnished Midland Tube-Line material are Guyen Alloys, Liberty Equipment, and W J Woolley.
3.
All Tube-Line ma+ trial identified as being furnished to Midland has been entered on nonconformance reports.

a) Nonconformance Report C-00355; identifies 50 fittings received from l
Guyon Alloy and Liberty Equipment, 17 fittings are located in Warehouse No 1 in the separated hold area with hold tags attached.
b) Nonconformance Report C-00961; identifies nine fittings from Guyon Alloy and Liberty Equipment that have been incorporated into Q systems. All nine fittings have been located and a hold tag placed on l
each fitting.
c) Nonconformance Report C-00915; identifies two 10" caps from W J Woolley that have been incorporated into ASME Class-2 Air Tank OT-921. Hold tags have been attached.
t These three Nonconformance Reports are scheduled to be dispositioned by June 1, 1984.
A final report on IE Bulletin 83-06 is planned for June 15, 1984.
