REPLY TO NOTICE OF VIOLATION INSPECTION REPORT NO. 50-483/95008 CAILAWAY PLANT This responds to Mr. M. J. Farber's letter dated July 17,1995, which transmitted a Notice of Violation for events discussed.in Inspection Report 50-483/95008. Our response to the violation is presented in the attachment.

- 5 Attachment to ULNRC-3246 Page 1 of 2 Response to Viointion 50-483/95008 Statement of Vlointion During an NRC inspection conducted April 30 through July 1,1995, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedures for NRC Enforcement Actions," (60 FR 34381; June 30,1995) the violation is listed below:
Callaway Plant Technical Specification 6.8.1 states, in part, that written procedures shall be established, implemented, and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.
Regulatory Guide 1.33, Appendix A,1.g requires administrative procedures be written for Shift and Relief Turnover.
Operations Department Procedure ODP-ZZ-00003, Revision 10, " Shift Relief and Turnover," step 4.3.2 states, " Prior to relief, the relieving RO and BOP operators shall: Review the pertinent information in the RO logs, for the last three (3) days or since his last watch whichever is shorter."
Contrary to the above on May 19, May 22, and May 23,1995, the NRC inspectors observed three reactor operators (ROs) and three balance of plant (BOP) operators relieve the watch without reviewing the log for the last three days or since their last watch. (483/95008-01(DRP)).
This is a Severity Level IV violation (Supplement I).
Reason for the Violation Inattention to detail and oversight by licensed operators to complete the turnover requirements of ODP-ZZ-00003.
Corrective Steps taken and results achieved:
The requirements of ODP-ZZ40003 were reviewed with the licensed operators on each crew. Management expectations for adherence to these requirements were clearly communicated to all involved personnel.

Corrective steps to avoid further vialations:
Management's expectations regarding procedure adherence were discussed with shift 3
crew supervisors at a monthly staff meeting and with licensed personnel during the management expectations presentation in Requal Cycle 95-3 or while onshift.
Additionally, the Superintendent-Operations has stressed to all shift supervisors that j
they are responsible for the quality of the licensed operator shift turnovers. The l
Superintendent-Operations and a shift supervisor have monitored turnovers to ensure adherence. Procedure ODP-ZZ-00006, " Operations Department Routine Logs" will be l
revised to maintain all required narrative log entrys on the Reactor Operator and Senior Reactor Operator clipboards.
Date when full comnliance will be achieved; Full compliance was achieved May 23,1995. Corrective actions will be complete August 25,1995.
