This Interim Report is in reference tc the pot ent ially report ab le Significant Deficiency relating to the wiring workmanship on two Gould 480 V Motor Control Centers. An evaluation is currently underway to deter-mine the report abili ty and impact of this problem fo r BV-2.

BEAVER VALLEY POWER STATION - UNIT NO: 2 DUQUESNE LICHT COMPANY Report on Potential Significant Deficiency No. 84-02 480V Motor Control Centers 1.

SUMMARY

The BVPS-2 Site Quality Control (SQC) group has repo rt ed ins t ances of what appear to be poor workmanship practices found in factory ins talled wiring in the Class 1E 4160V switchgear and 480V unit subs t at io ns furnished by Brown Boveri ( B B) and the 480V n.ator control center (MCC) furnished by Gould.

This equipment was supplied under purchase orders 2BV-304, 2BV-307, and 2BV-310, res pe ct ively.

The ins t ances have be en r ec orded in several Nonconformance and Disposition Report s and ot he r limited cursory inspect ions that have been prepared by SQC.

IMMEDIATE ACTION TAKEN Site ins pe ct ions of the 4160V switchgear, the 480V unit subs t at ions,

and the 480V MCC's were condeted on a random basis by the SWEC equip-me nt s peci ali s t and a BB represent ative.

These ins pe ct ions confirmed that the equipment did exhibit ins tances of poor workmanship pract ices and also confirmed the need for further analysis.

DESCRIPTION OF THE PROBLEMS Specific ins tances reported include:

1.

Couged, cut, or chafed insulation on switchboard wire.

2.

Conductor strands not captured in terminal lug barrel.

3.

Broken or nicked conductoe s trand s in switchboard wire.

4.

Taunt powe r cable connect ions from the powe r s t ab s to the line terminals of the breakers ins talled in the MCC's.

ANALYSIS OF SAFETY IMPLICATIONS The observed conditions, without further analysis, have been judged to have a potential to cause an electrical failure of the equipment during a seismic event.

Because the problems noted above were observed on redundant switchge ar,

unit subs t at ion, and MCC's, and because this eq uipment provides powe r for the majority of the plant's safety related systems, the problems described above could result in the loss of plant capability to safely shut down.

5.

CORRECTIVE ACTION TO REMEDY DEFICIENCY Ins pect ions will be pe rformed and any deficiencies noc ed during the inspections will be analyzed and, if necessary, repaired to assure that the functional integrity of the equipment is maint ained.

6.

ADDITIONAL REPORTS An additional report will be issued by July 1,1985.
