Enclosed is Licensee Event Report (LER)95-003, Revision 00, for Quad Cities Nuclear Power Station.
This report is submitted in accordance with the requirements of the Code-of Federal Regulations, Title 10, Part 50.73(a)(2)(v)(B).
The licensee shall report any event or condition that alone could have prevented the fulfillment of the safety function of structures or systems than are needed to remove residual heat.
i The following commitments are being made by this letter:
1 Procedure QCOP 1000-5, Shutdown Cooling Operation, will be changed to specifically require the depressing of the reset pushbutton for the M0 1001-29A(B) prior to operation of the valve. -This will be completed by September 30, 1995.
Procedure QCOS 1600-13, Refueling Outage PCI Groups 2 and 3 Isolation Test, will be changed to ensure that the proper reset of all isolations in'the procedure are accomplished.
In addition, a statement will be included in the procedure to ensure that the reset ste.ns of the logic are included if performing a partial test. This will be completed by September 30, 1995.
Procedures QOP 7000-1, Reactor Protection System MG Sets, and Q0A 7000-1, 120 VAC Reactor Protection Bus Failure (One or Both Buses), will be changed to require the reset of M0 1001-29A and B by depressing the pushbutton.
This will be completed by September 30, 1995.
A review will be performed of all other procedures, that insert sealed in signals to valves that are susceptible to rapid reversal.
The purpose of this review is to insure that the signal is properly reset prfor to stroking these valves.
This review will be completed by September 30, 1995 and any necessary procedure changes completed by November 30, 1995.
