Dear Mr. Murley:
The following is our complete response to NRC IE Bulletin 83-06 concerning possible nonconformities of Tube-Line Corporation supplied ma terial. To completely respond to the Bulletin, Bechtel Power Corporation reviewed 415 vendors who provided material to various Bechtel nuclear proj ec ts.
The review identified 30 vendors who may have supplied '1bbe-Line material to the Limerick Generating Station.
It ms confirmed that only" one supplier, Guyon Alloys, Inc. of Harrison, NJ, delivered ASME
.. material from Tube-Line to the Limerick jobsite. A list of the material delivered from Guyon is presented in Att.1chment 1.
Tube-Line Corporation notified Bechtel (Attachments 2 & 3) that material received from Guyon under five purchase orders may not have been fabricated to ASME Section III requirements. The material identified included:
1.
An eight-inch blind flange (Attach.1, Pg. 2, Order 8031-F-57228).
2.
Ten six-inch 45 degree elbows (Attach.1, Pg.1, Order 8031-F-4 5718).
3.
_ Two four-inch S.R. 90 degree elbows (Attach.1, Pg.1, Order 8031-F-45829).
4.
Seven six-inch by three-inch concentric reducers (Attach.1, Pg. _2, Order 8031-F-48026, Item 3).
5.
A four-inch L.R. '90 degree elbow (Attach.1, Pg. 2, Order 8031-F-48216, Regn. 73881).

The eight-inch blind flange, the six-inch 45 degree elbows, and the four-inch S.R. 90 degree elbows (Items 1, 2, and 3) are still at the jobsite. In a letter to the NRC dated June 9,1983, Tube-Line verified that the eight-inch blind flange was properly heat treated per ASME SA-182 requirements and was suitable for use. The ten six-inch 45 degree elbows and one four-inch S.R. 90 degree elbow have been quarantined per NCR 7375 and are awaiting shipment back to Tube-Line. The other four-inch S.R. 90 degret elbow has been installed in a non-safety rel.ated system and has been accepted for use by the engineering disposition given on NCR 7375. Items 4 and.5 were returned to Tube-Line and have been replaced with stainless fittings supplied by Q21f and Western-Taylor.
The other items listed on Attachment I have also been returned to Tube-Line. These items are:
1)
Twenty Efive four-inch by two-inch concentric reducers (Attach,1, Pg. 2, Order 8031-F-48026, Item 1) 2)
Ten three-inch caps (Attach.1, Pg. 2, Order 8031-F-l.R216, Regn. 73387.
All other Tube-Line supplied ASME material has been accepted for use in accordance with Bechtel Specification P-309.
This material includes a three-inch 45 degree elbow (Attach 1, Pg.1, Order 8031-F-45718, Item 6) which has been installed in the Standby Liquid Control Sys tem.'
The remainder of the Tube-Line ASME ma terial accepted under P-309 has not been installed and has not been assigned to a system.
In' m2mmary, except f or the elbow installed in a non-saf ety related system, all Tube-Line material delivered to Limerick Generating Station
- has been 1) manufactured per ASME code requirements, 2) replaced with material that has been manufactured per ASME code requirements, or 3) will be returned to Tube-Line. The above statement concludes our response

We shipped to you pursuant to purchase order issued by you to Guyon Alloys the stainless steel flanges listed on the attahced Appendix A on the dates indicated.
We have recently initiated a comprehensive review of our Quality Assurance Program. This review has indicated that the items listed may not be heat treated in accordance with all of the requirements of ASME'SA-182.
We have initiated steps to determine whether the physical properties.of the items shipped to you differ in any material respect from items heat treated to ASME SA-182 We would like to meet with you as promptly as possible once the test results are available to evaluate them and to detemine whether any upgrading or replaccment is required.
In the meantime, we request that you ascertain the location of each piece involved and that there be no further installation of any pieces currently under r.eview.
We are undertaking this effort to ensure that any potential safety Problem is addressed at the earliest possible time and because we value our reputation as.a supplier of high quality products.

We have recently initiated a comprehensive review of our Quality Assurance Progress. Viis review has indicated that the itano listed uhich were made freus A-240 plate and which appear to have been welded with filler metal were not manufactured in accordance with all of the requirements of sh-403 and section III of the ASME Boilar & Pressure vessel Code.
Vie particular requirements to-volved concern NPT symbol stamping, verification of welding procedures and welder qualifications, verification of welding work by an authorised inspector, receipt of Data Reports f ams the manufacturer, and verification of the chemical properties of the weld metal used in the fittings under review. De have tattisted steps to detenaline the chemical properties of the weld metal and whether any of the fittings were welded without filler metal.
We are sending this notice to you pursuant to 10 C.F.R. 921.21(a) and have provided a similar notice to Guyon Alloys, Inc.
