An evaluation is currently underway to determine the impact of the undersized welds.

! Report on Potential Significant Deficiency No. 84-01 Clamp Anchor Assemblies with Undersized Welds J1.
SUMMARY
p Beaver.~ Valley Power Station Unit 2's' (BVPS-2) Mechanical Contractor, E SchneiderJ Power Corp., had ' the. res pons ibility fo r the fabrication of l
clamp anchor assemblies i for small-bore pipe.
These were initially to i
b4 be' fabricated in the contractor's fabrication facility local to the site that is : subjected to similar controls to those placed upon fab-
~
rication act ivities at 'the site itself.
However, the contracto r
~ reques ted and. obt ained ~ approv al to sub-contract these assemblies - to Pittsburgh Bridge L and' Iron (PBI) for manu factur e.
During a routine
'p' finspection off installed ' piping on site, a QC Inspector who did not have
-; direct -responsibility for the inspection of the clamp anchor. assemblies reported that he considered the welds to be undersized.

- On : receipt of the QC Inspector's report, an inspection was performed on I'

, plant, but' had -- no' opportunity to independently verify a sample of.
the - welding.
They were not given shipping rele ase au t ho ri ty,
'usually: applied under normal purchasing activity.
' d. 1The ~ material. supplied. to the vendor by the Mechanical Contractor c wastof marginal size' relative to - the vendor being able to meet the s
weld leg size, and Jthere appeared to be no special co9rols during
.the' manu fa'cture to assure the _ centering of the channel on the flat
! plate. - The centering of the ' channel on the plate controls the weld leg sizes in question.
~ 4. ' ANALYSIS OF SAFETY IMPLICATIONS BVPS-2is ' currently evaluating the weld size condition as obt ained and
.in addition is performing a series of laboratory testing to demons trate
. the strength of: the -assemblies.under loading conditions. Therefore, at
. this time, the safety significance of the condition is not known.
- 5. -CORRECTIVE ACTION TO REMEDY DEFICIENCY
'The corrective action to remedy the deficiency will ' to some extent be dependent 'upon the. result s of - the tes t ing and evaluation performed by
-BVPS-2. 'However, prior to the lif ting.of the Stop Work Order currently imposed upon the vendor -for the production of these assemblies, action will be taken to. assure that-_ the material supplied for fabrication is
. of suf ficient size, and manufacturing controls are in ef fect,- to obt ain
- the required ' weld _ leg size.
BVPS-2 Ve ndor Surveillance Group will
_ perform sample inspections on a regular basis to es tablish the ef fec-
.tiveness of _ the vendor's--QC. program, and will be given shipping release authority' for each shipment prior. to delive ry.
In the event that the.
l tes ting - program and evaluation - indicates a safety significance, the appropriate - repair ' or removal-of the items will be ;e rformed.
A program _it, ' being ~ ins tituted to es tablish the location 'of each clamp anchor assembly _at the site.

ADDITIONAL REPORTS An additional report. on this Potential Significant Deficiency. will be issued af ter1 : the.~ results ' of the [tes t ing _ and evalua t ion pr ogram are
- obt ained and reviewed. This report will be submitted by Duquesne Light 2 Company by June.1,1984.
