On January 26, 1984 Northern States Power Company, Prairie Island t

Nuclear Generating Plant was notified by the NRC of a potential lapse of quality control at the Hills - McCanna, Ramcon Actuators and Controls Division of Pennwalt Corporation (presently owned by Rockwell International).

An investigation was begun to determine the applicability of this issue at Prairie Island.

It was determined that P1 has 86 of the aforementioned actuators installed in safety related ventilation systems.

(See attached Table 1 for system identification and location.)

The actuators were supplied eith dampers as a unit from Pacific Air Products Company (PAPCO) in accordance with Pioneer Service and Engineerir.E Compan-/ (presently Fluor Engineering, Inc.) specifications Hiawatha 505, M1282 and M1283 and NSP Quality Procedure D-9 " Quality Assurance Requirements for Fabrication, Construction, Installation or Testing."

The following requirements for the actuator were imposed upon PAPCO:

1.

The time required to actuate air operated dampers shall not exceed 10 seconds.

Motor operated dampers shall open in 30 sec (+10, -5).

All damper operators shall be suitable for mounting in any position and be air to open and spring to close or spring open and air to close without positioners except as noted.

Shield Building Vent dampers shall have modulation capability (DC motor operated).

3.

Clean dry instrument air will be furnished by NSP at 85 psig.

4.

Operators shall be selected which shall close the damper within the specified time limit yet provide for a cushioned seating so as not to damage the seals.

5.

Items designated as Quality Assurance Type 1 and 11 shall be analyzed for seismic accelerations.

The

' analysis shall be made on the assembly (damper and actuators) as a whole.

6.

Subcontractors and suppliers shall be selected

_ primarily on the basis of demonstrated capability to provide a product, process or service in accordance with the design, specifications and contract provisions.

An evaluation of subcontractor capability shall be required on items that cannot or will not be fully inspected upon receipt, on engineered items, and on standard items embodying significant changes in size, perfo rmance, configuration or application.

Contractors, subcontractors and suppliers shall maintain records of their suppliers capabilities and quality performance.

Where valid records are not available, an on-site survey of the supplier's facilities, capabilities, personnel skills and quality i

assurance system will be made.

The product delivered to Prairie Island met the above requirements as stated in multiple certificates of conformance from PAPCO.

Damper / actuator operability was again verified after installation.

NSP has been informed by Rockwell International that a nuclear quality assurance program and a commercial quality assurance program were intact at Hills-McCanna at the time the Prairie Island actuators would have been manufactured; Prairie Island's maintenance history further justifies the acceptability of Hills-McCanna actuators for safety related applications.

Table 2 exhibits repairs made because of actuator failures.

It should be noted that the actuator failure rate is approximately one failure per 100 actuator years in service.

All failures have occurred on the containment fan coil unit dampers which is the most severe service.

A failure has not been experienced in the-last five operating years.

This excellent operating history can be attributed to successful preven 6Chaintenance (Table 3) and the following modifications:

1.

Grease fittings have been added to the actuators to allow lubrication of the shaft and bearing sleeve.

2.

The air supply to each fan coil unit damper actuator has been orificed to reduce opening and closing thrust.

3.

14 Containment Fan Coil Unit Discharge Dome Damper CD-34078 was repositioned so that turbulent flow which causes excessive vibration would not be produced.

4.

A dry instrument air supply is now available to prevent internal actuator corrosion.

5.

A new yche design by liills-McCanna has been incorporated to prevent failures.

6.

Air line lubricators have been installed to Lalp alleviate the stem-bushing corrosion problems.

January 30, 1984 Page 4 of 5 Scheduled surveillance routinely verifies the operability of the associated actuators by cycling them to and from their safeguards positions.

The associated surveillance procedures and frequencies are displayed in Table 4.

Any data on surveillance procedures which is indicative of any abnormal conditions is acted upon and reviewed promptly at Prairie Island.

Past experience and maintenance has proven the actuators acceptable in their present installation.

The

damper / actuator specifications, including seismic response requirements and quality assurdnce requirements have been met by Pacific Air Products Company.

The Hills-McCanna actuators provide no additional risk to the safety and health of the public as compared to any other actuator.

To assure the integrity of the actuators at Prairie Island Nuclear Generating Plant is maintained Northern States Power Company concedes to perform the following:

1.

Inspect containment fan coil unit dampers locally, including damper alignment with actuator and verify smoothness of operation at refuelings.

2.

Continue with the existing surveillance program as directed by the Technical Specifications On 1-30-84 Prairie Island Operations Committee reviewed the maintenance history including preventive maintenance, modifications, l

present surveillance and the proposed additional inspection and surveillance. The Committee judged that the l

actuators and the systems which utilize these actuators are operable.

Based on their judgement we conclude that the health and safety of the public is not compromised with continued operation.

Based on our present knowledge and discussion with the new vendor, Rockwell International, we feel a QA program existed at Ilills-McCanna when the actuators were manufactured initially for Prairie Island.

We also feel that our continuca programs af preventive maintenance, surveillance and inspection, and corrective action when a problem is identified, serves well as a continuing qualification program.

Should.we find deficiencies that. existed with the program comprising quality,,we will take steps to resolve the deficiencies.

These actions will be completed by 5-1-84.
