Removal of a second cooling tower from service within the allowable 48-hour maintenance period.

Conditions Prior to Deviation:

With the 'C' cooling tower out of service for scheduled main-tenance (less than 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br />) the

'A' cooling tower was removed from service due to a gross leak on the distribution header.

Designation of Apparent Cause.of Deviation:

Failure of the banding around the distribution header which resulted in the blowout of a portion of this wooden structure.

Since the "A" to,wer was out of service for only 30 minutes, there were no significant environmental consequences as a result of this deviation. River flow during this period was in excess of 45,000 cfs.

Corrective Action:

The damaged wooden planking was replaced and secured by a stainless steel plate and the tower was returned to service.
