Dear Sir:

N

-l As defined by Technical Specifications for the Vermont Yankee Nuclear Power Station, Section 6.7.B.d, we are reporting the following Abnornal Occurrence as A0-72-17. ,

At 0445 on December 22, 1972, during conduct of the monthly HPCI pump operability surveillance test, the gasket on the gland seal exhaust condenser failed. 'This cooler is required for proper operation of the HP,CI and its loss temporarily precludes the HPCI operability required by

'Section 3.5.E.1,a of Technical Specifications. The reactor was operating at approximately 62% power at the time of this incident.

Replacement of the failed gasket was initiated immediately. Concurrently, the operability verification required by Section 4.5.E.2 of Technical Specifications was initiated. The cooler was repaired and HPCI was verified operable at 1125 on the date of the occurrence.

} / The Plant Operations Review Committee concluded that the cause of gJ jthe blown gasket was an overpressurization of the condenser due to an improper j i / setting of the HPCI PCV23-50. The pressure control valve was reset and locked j in a position to prevent valve opening at a pressure greater than 40 psi and k thereby prevent an overpressure condition in the condenser.

IIT jg Very truly yours,

)
