Identification of Geviation:

Two cooling towers simultaneously inoperable.

Conditions Prio> to Deviation:

Unit 3 operating at 70% power. Unit 2 off for refueling outage. The "A" tooling tower was out of service due to a previous lift pump bearing failure. The "C" cooling tower was fully operabic during the outage. River flow was about 39,000 CFS.

Description of Deviation:

T An operator making his rounds saw water gushing from the door of the "B" lif: pump structure. The pump-was removed from service making two coollag towers inope.:able.

Designation of Apparent Cause of Deviation:

Bolts holding the suction bell housing to the pump casing f ailed, causing the housing to fall ~to the bottom of the lif t pump pit.

T The lif t pump discharge piping was cracke'd, and the base plate of the pump had shifted.

Analysis of Deviation:

N Differential temperatures between the Pennsylvania-Maryland State and Holtwood Dam remain within Pennsylvania _ State limits. Single unit operation during this period minimized the' thermal impact of this deviation.

.,C_orrective Action:

Investigative work is continuing to determine the exact cause of failure. Expidited maintenance work !s in progress to return the lif t pump to service as promptly as possible. Philadelphia Electric Company Engineering Department and several consultants are involved in a co-ordinated effort to identify the cause of this failure and defermine a course of action to prevent recurrence of this type f ail-N ure.
