Fort St. Vrain, using procedures and supervisory personnel supplied by

General Atomic.
A plug was inserted into the fuel hole which was found E

to be without one, using a procedure provided by General Atomic.

s These discrepancies are being investigated by General Atomic as to the y
cause of the damage and as to why their Quality Assurance procedures failed to find them.
General Atomic has been requested to provide Public

Service Company with full information.
