Abnormal Occurrence as A0-73-03 and confirming our telephone conversation E

with your office.

Q At 2350 on January 15 during RHR surveillance testing, "A" Loop injection Valve (RHR-25A) failed to open, rendering the "A" LPCI E

subsysten inoperable. This inoperability of "A" LPCI subsystem exceeds i

the Liniting Condition for Operations as defined by section 3.5.A.1 of Technical Specifications. The subject section requires both LPCI

subsystens to be operable whenever irradiated fuel is in the reactor vessel. The reactor was operating at 1D0% power at the time of this incident.

E Initial investigation revealed that the valve notor was unusually warm and that the notor breaker had tripped. The surveillance required s

1 by'Section 4.5.A.4 of Technical Specifications was conducted.
