Dear Sir:

At approximately 1100 on June 12, 1973, while performing a calibration l in accordance with Tech. Spec. Table 4.1.1, the First Stage Turbine Pressure Permissive Pressure Switch 5-14A, which deenergizes relay 5A-K9A above 220 psia, failed to actuate at the proper setpoint. This sensor is required by Tech. Spec. Section 4.1. A to operate with the reactor mode sw!tch in the RUN mode. The reactor was operating at approximately 75% at the time of this occurrence.

The switch was reset f rom 235 psia to operate at a conservative setpoint of 207 psia and returned to service at approximately 1120 June 12, 1973. This is a Barksdale switch supplied by General Electric Co. and was part of an overall review of Barksdale switches conducted by GE in late 1972. We 'were to have been informed as to the results of review but to date have received no information relative to the reported problem.

The Plant Operations Review Committee reviewed the reported incident and recommended that the surveillance test be conducted again in three months rather than six months as required by Technical Specifications.
