L/ l BRANCH W I LOG.<EEP.NG 1.0 PURPOSE This procedure provides guidance for keeping operations narrative logs, round sheets, operating logs, recorder charts, co=puter printouts, and special logs.

2.0 NARRATIVE LOG ENTRIES 2.1 GENERAL 2.1.1 Logs should provide an accurate history of plant operations as a narrative sequence of events or functions perfor=ed.

2.1.2 Logs shall be kept current. Infor=ation should be pro =ptly recorded. Entries should be clear, concise, cocplete and legible.

2.2 SHIFT SUPE 2 VISOR LOG 2.2.1 The Shift Supervisor Log should have an entry prefaced by the ti=e of occurrence, for activities on shift including:

a. The name and posit. ion of each operator on shif t,
b. Major equip =ent status changes, .
c. Major syste= and equip =ent testing,
d. Personnel injuries, .
e. Entering and exiting a technical specification action state =ent,
f. Significant events, such as reactor trips or unexpected power changes,
g.  !=ple=enting the E=ergency Plan, d
h. Significant security incidents, E
i. Mode changes.

VEGP 10001-C 10 2 of 15 Ll 2.3 UNIT (1 OR 2) CONTROL LOG 2.3.1 Entties may be cade by the Shift Supervisor, Reactor Operator, or Balance of Plant (BOP) Operator, preceded by the time of entry.

2.3.2 The Unit Centrol Log sho21d have an entry for any:

a. Mode chany;;,
b. Loed changes ,
c. Reactiv!.ty changes (other than during startup and shut down),
d. Equip =ent status changes,
e. Performance of surveillance testing,
f. Releases of radioactive effluents including: start and stop ti=es,
g. Out-of-specification chemistry results,
h. Operation of PORVs and opening of primary system ,

and significant secondary system safeties.  :

2.4 OPERATOR LOGS 2.4.1 Operator logs will be kept for local operating stations c as deter =ined by the Operations Superintendent.

Entries should be cade by the operator on duty at the local station.

2.4.2 Operator logs should have an entry, prefaced by the time of occurrence, for the following as applicable:

a. Major equip =ent or syste=s changes in operating status,
b. Equip =ent failures or problems, including follow-up corrective action,
c. Radioactive vaste discharges, start and stop,  ;
d. Operating station specific events. 1 9
