Engine K-9204-X (B-Engine of 1A Standby diesel generator) became inoperable due to an air-locked fuel system on May 16, 1975.

This is identified as an Abnormal occurrence per Technical Specification 2.1, Paragraph (f).

DESCRIETION OF OCCURRENCE:

"B" Engins was being tested to ensure that it timed out in 45 seconds cranking time if it failed to start. This is an annual test of the timing instruments.

To ensure the engine would not start the actuator output was turned to zero.

After the 45 second cranking was complete, the actuator was reset. The engine however, failed to start, because the fuel system was air-locked.

Procedure Cranking of the engine at any time with the fuel shut of f introduces the possibility of air-locking the fuel system.

ANALYSIS OF OCCURRENCE:

This is an abnormal occurrence because the engine was not removed from service, and logged as such, during the routine testing.

The engine was, in fact, out of service as soon as the fuel was shut off.

No cicarance was required, so there was no logged indication of the engine being inavailable.

This appears to be a violation of paragraph (g) Section 2.1 of the Technical Specifications.

CORRECTIVE ACTION:

  • 1he procedure for checking "overcrank" protection will be developed so that 1.

A log of dicac1 engine unavailability will be kept in the Shif t Supervisors 2.
