Report of 10CFR50.55(e) Its on Unacceptable Contractor Practice regarding Ultrasonic Testing of Flued Head Structures (#100)

Dear Mr. Keppler:

On September 9,1983, Detroit Edison's Mr. W.R. Wingfield, Quality Engineer - Construction Quality Assurance telephoned Mr. J. Konklin of NRC Region III to report on unacceptable contractor practices regarding Ultrasonic Testing (UT) of Flued Head structures.

A problem was identified concerning a site contractor's (Wismer and Becker) Ultrasonic Testing practices. The problem is inconsistent implementation of the UT procedure, inconsistency in technique utilization, and a deficiency in their Quality Control program by which known deficiencies were not reported in a timely manner.

This has affected remedial and preventative corrective action.

(i.e. Rejectable UT reports over one and a half years old had not been processed, nor had the rejectable item been repaired) .

The extent of the problem, and the safety implications have not yet been determined. An investigation is continuing.
