I This letter is to report information concerning the station's E

second inspection of Bergen-Pctorson hydraulic shock suppressors on Unit #3.

The unit cas chutdown on October 27, 77 dcyc af ter rhe initial snubber outege, to perform a cecond inspection as required by Directorate of Regulctory Cperctions bullocin 73-4.

The attcched list contains piston rod extensions and fluid level indica to r positions found during this incpection.

The inspection revealed that 41 Eergen-Paterson snubbers on Unit #3 were operabic, and er.hibited overall integrity. There were two snubbers in the drywell (I.D.#26 and 27) that had no oil level indication on the snubber accumulator. They were removed frcm the drywell for inspection and repair. The subecquent disassembly of these two snubbers revealed such a low oil icvel that it is questionable that they were operable.

Seven of the operable snubbers had low oil level indication on the snubber accumulator, but the oil level was not low enough to consider them inoperable.

Three of these seven (I.D.#9,12, and 25) had significantly lower oil level than the others and were removed from the drywell for more thorough inspec-tion and testing.

It is believed that the low oil level in the other four anubbers (I.D.913, 28, T7 cnd TIS) resulted frem improper filling during the initial inspection end overhaul, since no fluid lecks were evident.

The oil level in these four enubbers was increoced prior to resuming power operaeion.

Mr. James C. Keppler November 2, 1973 Snubbers #25, 26, end 27 were dicassembled, repaired and tested satisfactorily prior to returning them to their original locations.

l Incpection of snubber #25 revealed that the biced screw '*0"-ring and most i

of the "O"-rings on the ennifold coce=bly were brittic and broken.

In addition, the head ceal vac ficttened cnd broken.

Inspection of cnubber #26 revealed that the head cent was flattened, that the cenifold "0"-ring was broken, cnd that coct of the secle in the = nifold accc=bly were brittic, but intcet. The oil level indicator wcs broken durin; dicacccmbly chich necccciteted repicecuent of the indicator picton and the cecu=uletor cylinder.

In cddition, the piston red cylinder, the pisten ring: cnd the accuculator spring were replaced.

Inspection of snubber #27 revealed a faulty heed ceal brittle seals in the conifold ecoc=bly, with cc=c broken, cnd a c=all scratch on the piston rod cylinder.

It could not be determined whether the broken "0"-rings in the three snubbers that ecre overhculed were broken during disaccenbly or were actually broken during routine operatien.

Since no oil Icekage wcs discovered in the examination of snubber #9, it vac filled with oil, tected saticfcetorily, cnd returned to the drywell.

Inspection of cnubber #12 revealed a demaged "0"-ring on the biced screw. Following replacement,o'f the bleed screw "0"-ring, the snubber was filled with oil, tected satisfactorily, and returned to its original location.

All seals removed were untreated polyurethene and all new seals installed were ethylene-propylene, with the exception of the accumulator seal and the piston rod packing washer, which were untreated polyurethane.

Eleven Grinnell snubbers, located in the turbine and isolation condenser pipeways.were incpected to supplement the above deceribed inspection. All wcke operabic, but one of the Grinnell enubbers had low oil indication.

It was filled with oil cnd will be repaired or replaced at a 1cter date due to its non-safety related function.

This program l;

is being developed through the efforts deceribed in our letter of October 15, 1973 concerning the Unit #2 inspection.
