Gentlemen:

.Recently, Entergy Operations, Inc. became aware timt material had inadvertantly been deleted from the Waterford 3 Emergency Plan when Revision 15 of the plan was issued.

Briefly, Table 4-1 of Revision 15 to the plan, " Summary of In'tlating Conditions," does not include the initiating conditions related to toxic chemleal eme rgencies. Since the (incomplete) table is not used to classify emergency conditions, this omission represunts an administrative oversight only and not an operational concern. The two procedures used to classify emergency conditions and initiate plant responses, Emergency Plan Implementing Procedures EP-001-

001, " Recognition and Classification of Emergency Conditions," nud EP-004-010,

" Toxic Chemical Contingene; Procedurn," are not affected by the omissions from Table 4-1. Therefore, no plant response to any emergency condition is affected by this omission. Although the incomplete table is not significant from an operational standpoint, this letter is provided to confirm Entergy Operations, Inc.'s intention to revise the Emergency Plan to include the deleted material by November 27, 1991. These plans were discussed in conversations between Waterfo-d 3 personnel and members of the NRC staff on October 16, 1991.

Table 4-1, " Summary of Initiating Conditions," was completely revised in Revision 15 of the Waterford 3 Emergency Plan. The intent of the changes was to replace the generalinformation previously included with more specific initiating conditions.' During this process, the initiating conditions related to toxic chemical events were inadvertently omitted.
