The attached, Interim Report No. 2, is in reference to the potent ial for misoperation of DS-416 reactor trip switchgear undervoltage at t achme nt s reported to the Nuclear Regulatory Camission by S. D. Hall of Duqmsne Light Company (DLC) on April 26, 19 83.

Wes t inghouse has es t ab lished a task fo rce to evaluate this pr oblem.

When the result s of this evalua t ion are ava ilab le, DIE will issue anot her report on this subject.

It is expected that this report will be submitted to the Region I of fice by January 9,1984.

SUMMARY

On March 30, 1983, Wes tinghouse (W) advised af feeted opetating plant s of the potential for misoperation ofI)S-416 reactor trip switchgear undervol-t age at t achment s based on repo rt ed mal f unct ions at one pla nt during testing.

W advised operating plants on April 15, 1983, of an additional misopera-t ion of another DS-416 undervoltage at t achme nt.

Inves tigation of this eve nt reve aled a missing retaining ring on one of the two undervolt age attachment pivot shafts.

Brunner, Region I of fice, on April 26, 1983, that an evaluation of this problem for Beaver Valley Unit No. 2 (BVPS-2) was underway.

DESCRIFTION OF DEFICIENCY Based on repo rt ed mal funct ions at one plant during t es t ing, W advised af fected operating plants on March 30, 1933, of the pot ent ial fo r misope r-ation of DS-416 reactor trip switchgear undervolt age attachment s.

W began an evaluation of the input from several af fected operating pla nt s con-j cerning dimensiores of several clearances in the undervoltage attachment.

I On April 15, 1983, W advised operating plants of an additional misopera-t ion of another DS-416 undervoltage at t achment.

Investigation of this event revealed a missing retaining ring on one of the two undervolt age a t t achme nt pivot shafts.

This allowed the pivot shaft to move laterally such that one end came out of its guide hole in the frame of the undervol-tage attachment, and did not permit the attachment to operate on demand.

4.

ANALYSIS OF SAFETY IMPLICATIONS W evaluated the input from several af fected operating plant s concerning dimens ions of seve ral c le arances in the unde rvo lt age at t achme nt ani concluded that devi at ions from the recommended clearances could increase the pot ent ial fo r miso pe ra t ion of the a t t achme nt, the reby creating a condition wherein the reactor trip switchgear might not open on automatic demand from the reactor protection system.

The W evaluation of the retaining ri ng issue reve aled a discrepancy in design.

The gr oove in che shaft receiving the retaining ring was not l

increased in width to be consistent with an earlier (1972) retaining ring

design ch ange.

The new retaining ring is wider than the original design and does not seat prope rly in the existing gr ooves.

This disc repancy increases the pot ent ial fo r misope ra t ion of the DS-416 unde rvo lt age

a t t achme nt, thereby creating a condition wherein the reactor trip switch-gear might not open on automatic demand from the reacto r prot ect ion system.

5.

CORRECTIVE ACTION TO REMEDY DEFICIENCIES Westinghouse has established a special task force to evaluate this problem and the corrective action required to remedy the deficiency.

BVPS-2 will issue another report on this subject when the results of this evaluation are available.
