On June 13, 1973, TVA made initial report to AEC-DRO Inspector W. S. Little of the malfunctioning of the governor on a Browns Ferry emergency diesel generator.

In accordance with paragraph 50.55(e) of 10 CFR 50, we submit the enclosed interin. report of the malfunction.

, , ,, l U ENCLOSURE U l BROWNS FERRY NUCLEAR PLANT INTERIM REFORT ON FAILURE OF GOVERNOR ON EMERGENCY DIESEL-GENERATOR B MODEL 990-20-645E4 On June 13, 1973, TVA made initial report to AEC-DRO Inspector W. S. Little of the malfunctioning of the governor on Brovns Ferry Nuclear Plant emergency diesel generator B. The following is an interim report on the malfunction.

When the governor did not respond to remote control, an immediate investiga-tion determined that the signal from the EGA box to the hydraulic actuator was out of the range specified by the manufacturer and the low-speed (idle)

setting of the hydraulic actuator varied over an 80-rpm range. A subsequent

! inspection revealed the following:

1. Small fragments of foreign material were found in the hydraulic actuator' 5

oil.

2. Two loose terminals were found on the electronic governor.
3. The set screws on the speed setting adjustment linkage within the governor actuator were not tightened completely. ,

Corrective action involves coordination among TVA, General Electric, Woodward Governor, and the Electro-Motive Division of General Motors, and this action is still in the process of being developed and coordinated.

i Another malfunction involving this governor was reported to Mr. Little on May 4, 1973. An interim report of that previous failure was submitted on .

. June 31, 1973. It was stated in that report that a final report was expected by July 31, 1973. We plan to cover both calfunctions of this governor in one final report and expect to provide it by July 31, 1973.
