I. APPLICABILITY, ENGINEERING DEPARIMENT NUCLEAR SERVICE DEPARIMENT PROJECT MANAGEMENT DEPARTMENT QUALITY ASSURANCE DEPARTMENT .

II. PURPOSE To provide a un'iform method for documenting problems associated with B&W supplied systems, components or equipment after shipment from the supplier's (B&W or other) shop, and to provide a timely resolution of site problems on all affected contracts.

III. EFFECTIVITY Thi's procedure is applicable to all Site Problem Reports initiated after the issue date of this procedure.

3. Site Problem Reoort Numbers - Each SPR is sequentially numbered, starting with 1 (one) for each contract. The full SPR number includes the contract l number, serial (sequential) number and revision number. For filing pur-poses, a Document Identifier of 13 has been assigned to SPR's. The assign-ment of SPR numbers is the responsibility of the Nuclear Service Suppodt  !

Engineer (NSSE).

C. Vendor Clain - A cost claim against a vendor. If a resolution of a Site Problem Report involved costs chargeable to a vendor, a Clain Report Work-sheet shall be prepared in accordance with NPG-1202-01.

D. Field Channe - A change to B&W supplied components or equipment to be implemented after shipment from the supplier's plant when any of the follow-ing conditions'exisc:

1. The change affects the interface, function or interchangeability requirements of a component or piece of equipment as defined by B&W requirements documents.
2. The change to B&W supplied equipment affects an interface between B&W and customar supplied equipment.
3. The change revises the as shipped design requirements of the com- F ponent or equipment ,

E. Problem Cross-Contract Accountability - A form (BWP-20141) used to identify other contracts and/or the Standard Product Line to which the problem may be applicable.

F. Servict Manager - A Nuclear Service engineer who assumes the NSS Project Manager's role as administrator of a contract once the contract has been declared commercial and Project Management turns control over to Operating Plant Services Section of Nuclear Service.

G. Problem Contact- Name of the person to coarset to obtain additional 1 infornation concerning the problem. ,

VII. GENERAL A. The Site Problem Report shall be used to report all site problems known to NPGD and to document cortective action. Corrective action shall be limited to restoring the equipment to its specified design or the indicatiou

, of a document (e.g. FCA, Site Instruction, etc.) to resolve the problem.

B. All SPR's and markings on attachments to SPR's shall be prepared in blue

! or black ink'or by typewritten.

If it becomes necessary to make any changes to technical content after one or more approval signatures have been obtained, the originator shall indicate the changes as stated above, and previous approvers shall re-approve by signature and redating to indicate their approval.

C. Revisiens to SPR's shall be limited to treating the initial proble= and*

shall be accomplished by:

1. Preparing a new SPR form and increasing revision level by one(l), or
2. Marking the revision on the existing SPR and increasing the revision level up by one (1). ,

In all cases, revisions shall require the same review and approvals as the original SPR.

D. Where time does not permit the orderly processing of the SPR before i=ple-mentatica, the resolution may be obtained via a Ta'X, telecopier or telephone provided the SPR is ice.ediately processed with the attached copy of Ta'X, telecopier or record of telephone conversation.
