FINAL REPORT SIGNIFICANT CONSTRUCTION DEFICIENCY NO. 54

" TUBE STEEL (HT 16600) LAMINATIONS" INTRODUCTION This report is submitted pursuant to 10CFR50.55(e) . It describes material lamination defects discovered on tube steel (HT 16600) used in the fabrication of hangers and supports. This problem is considered reportable under the require-ments of 10CFR50.55(e). To the best.of our knowledge, this problem has not been identified to the Nuclear Regulatory Commission pursuant to 10CFR21.

DESCRIPTION Mercury Company of Norwood, Inc. (Instrumentation Contractor), through issuance of nonconformance report W3-3304, identified that a section of tube steel (HT 16600) used for the fabrication of hangers and supports had material lamination defects. This (HT 16600) tube steel used by Mercury was identified in the installation of seven (7) safety related hangers and supports. Also, Tompkins-Beckwith, Inc., the piping contractor, utilized this (HT 16600) tube steel in the fabrication of twenty-six (26) hangers and supports for safety related piping.

SAFETY IMPLICATIONS The impact on safety related systems could have been significant due to support failure and subsequent degradation of safety related systems.

CORRECTIVE ACTION TAKEN Nonconformance Report W3-3304 was used to identify, track and document all corrective action. This NCR was closed on May 24, 1983.

As stated in the disposition of Nonconformance Report W3-3304, all tube steel with HT 16600 has been replaced with acceptable material and returned to Ebasco's warehouse for scrapping with the exception of 18 feet which is estimated to have been used up in the fabrication of miters and drop offs.

This report is submitted as the Final Report.
