Dear Mr. O' Leary:

The purpose of this letter is to provide you with further information concerning corrective action related to the referenced Abnormal Occur-ance Report, dated December 19, 1974.

Repairs to the !!0-2-202-4B valve commenced during replacement of the 2B Recirculation loop Discharge Valve Bypass Line.

Unit 2 was in the cold shutdown condition for refueling, and both recirculation loops were drained.

The 4B valve packing, operator, and yoke were removed, and the valve operator.was electrically removed from service. The packing glands and valve stem were thoroughly inspected.

Scoring was observed on the stem.

The middle packing stuffing box was out of alignment, causing the valve stem to bind. The scoring was evidence that the excessive binding was the cause for the valve not to fully close. This binding also led to the excessive electrical current necessary to close the valve.

Corrective action taken included the smoothing down of all scoring marks on the valve stem.

The valve was re packed. The motor rotation and torque switches were checked.

The valve was test cycled open and closed from the control room. The average opening current was 6.8 amps and the closing current averaged 6.3 amps during these tests. The repair work and cold functional tests-were completed by February 20, 1975 8306290680 750606 PDR ADOCK 05000265 S

On April 17, 1975, with the reactor pressure at 950 psig and temperature at 220 F, the H0-2-0202-4B valve was operated open and closed from the 0

control room. No abnormal valve operation was observed.
