DESCRIPTION OF NONCONFORMANCE During a CIO evaluation of QVP activities for PQCI C-1.90, several nonconforming conditions were observed.
For a description of these conditions see sheets two through four.
A total of three verification irs were reviewed and all three were affected by one or. more of the i
deficiencies contained in this report.

Sheet 2 of 4 Case No. 1 Drawing C-96 requires that nuts for stainless steel bolts shall be grade 8 or 8F. Contrary to this the following conditions exist which are not addressed on the applicable IR.
IR No. V-C-1.90-5 1)
Three nuts on the upper stem guide bracket and one nut on the lower stem guide bracket are grade 8M.
2)
One nut on the upper stem guide bracket, two nuts on the lower stem guide brackets, and all six nuts on the operator base plate show no grade markings.
IR No. V-C-1.90-4 1) hree nuts on the upper stem guide bracket and all four nuts on the lower stem guide bracket are grade 8M.
2)
No grade marking is visible on any of the six nuts on the operator base plate therefore the grade cannot be determined.
3)
One nut on the upper stem guide bracket and both nuts on the lower stem guide collar are coated and the grade cannot be determined.
IR No. V-C-1.90-3 1)
One nut on the lower stem guide bracket, both nuts on the upper stem guide collar, and all six nuts on the operator base plate have no visible grade marking, therefore the grade cannot be determined.
2)
_Three nuts on the lower stem guide bracket, both nuts on the lower stem guide collar, and all four nuts on the upper stem-guide bracket are coated so therefore the grade cannot be determined; Case No. 2 Vendor drawing No.
7220-C24-61'1 shows a 1%" x 6" bolt for attaching the frame extensions to the gate frame.
The affected bolts in gates OM 96A,'OM 968, and :0M91 A are 1%" x-6%".
These gates are' identified on the inspection reports listed below.

Case No. 3 PQCI C-1.40 requires that the Supplemental Data Sheet No.
I be completed for all bolts 7/8" and greater in diameter. Contrary to this the following conditions exist:

The stem guide bracket anchor bolts are not listed on the Supplemental Data Sheet No.1.
2)
The length of the bolts attad ing the frame extensions to the frame is listed as 6".
The bolts a e actually 6%" in length.
IR No. V-C1.90-5 The Supplemental Data Sheet No.1 indicates only one stem guide collar bolt as having no grade marking.
Actually two of the bolts have no grade markings. This condition is correctly indicated on NCR C-02789-V-TO.
Case No. 4 PQCI C-1.40 requires that the module number be entered on line 6 of the inspection report.
Contrary to this the following conditions exist:
IR No. V-C1.90-3 Module 800G is listed but the affected sluice gate is not in this module.
IR No. V-C1.90-4 Module 800F is listed but the affected sluice gate is not in this module.
A IR No. V-C1.90-5 Module 800G is listed but the affected sluice gate is not in' this' module.
Case No.'5 Corrections to irs are not initialed and dated on the following:

Case No. 7 The nuts used to attach the frame extensions to the wall are coated on all three gates affected by this report.
The nuts therefore could not have been inspected for proper grade.
The applicable inspection reports listed below do not address this condition.
