Dear Mr. Gagliardo:

This letter is written in response to your letter of February 29, 1984, which transmitted Inspection Report STN 50-482/84-05. As requested, the violation (482/8405-1) identified in the Inspection Report is being addressed in three parts.

a) The corrective steps which have been taken and the results achieved; b) Corrective steps which will be taken to avoid further violations; and

3) The date when full compliance will be achieved.

VIOLATION: FAILURE 'IO PROVIDE CONTROL OF ' HIE USE OF NYLON SCREWS IN ELECTRICAL TERMINATIONS Finding:

10CFR Part 50, Appendix B, Criterion V, requires that activities affecting quality shall be prescribed by documented instructions, procedurea, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instruction, procedures, or drawings.

Contrary to the above, there was no procedure to control the temporary use of nylon fasteners for electrical terminations

with permanent metal fasteners or to assure during this replace-ment, that all the electrical terminations were landed on cor-rect termination points.

Response :

a) Corrective steps which have been taken and results achieved:

Startup Administrative Procedure ADM 14-405, " Temporary Modification Control" has been revised to require the use of Temporary Modification Tags when nylon screws are to remain installed upon completion of Startup Testing.

This provides identification and traceability for removal prior to conducting Preoperational Testing. The practice of using nylon screws as circuit isolators by Construction has been terminated and is no longer authorized.

b) Corrective steps which will be taken to avoid further violations:

Startup and Construction hae developed a Two Phase Program to identify (Phase I) and remove (Phase II) all previously installed nylon screws.

During Phase I of the program the Construction group will physically identify by panel and termination the location of installed nylon screws.

During Phase II of the program, Startup will review the results of Phase I identification program and will replace the temporary nylon screws with permanent terminations in accordance with approved procedures. Additionally, twenty (20) other panels not identified in Phase I will be randomly selected and inspected to ensure the program identifies and removes all of the temporary nylon screws.

c) Full compliance will be achieved prior to Fuel Load.
