1, The original design was defective.
I;o. 2, the design engincers did not understand the extrenc serious strcsses due to the differences in tenperatures of closely connected areas.
I:y first inpression af ter nerely glancing over the report just roccived is very upsetting, Locause the size of the weakness cut in the 26" pipe.has been increased and the thickness of the safety valve flanges for the tenperature strain have also Leon increased.
These two things under some conditions of start ups or shut douns can cause troubic.
