Dear Mr. Walker:

INSPECTION REPORT NO. 50-4 83/84-04 (DE)

This reply is in response to your letter of April 3, 1984 which transmitted the report of the inspection conducted at Callaway Plant, Unit 1 during the period of January 30, 1984 to March 9, 1984.

Our response to-the item of noncompliance is presented below.

None of the material in the inspection report or in this response is considered proprietary by Union Electric Company.

(50-483/84-04-05) SEVERITY LEVEL IV VIOLATION 10CFR 50, Appendix B, Criterion V, as implemented by SNUPPS Quality Assurance Programs for Design and Construction, Section 17.1.5, and Startup Administration Instruction, SAI-5, requires that activities affecting quality be performed in accordance with documented instructions and procedures of a type appropriate to the circumstances.

Contrary to the above, the test results package for preoperational test CS-03BB13, RO, Pressurizer Relief Valve and PRT was reviewed and approved as evidence of proper operation of the system without adequately specified acceptance criteria for leak integrity of the Power Operated Relief Valves (PORVs).

In addiLion, one PORV was considered acceptable despite data which indicated to the contrary.

Corrective Action Taken And The Results Achieved:

On February 9, 1984, the Joint Test Group, during review of Procedure CS-03BB13, evaluated testing to meet the required test objectives.

At this time, it was noted that PORV PCV-455A had failed to meet the non-leakage objective and the item was entered g5gfofMp

Corrective Action To'Be Taken To Avoid Further Noncompliance:
