Inspection of Shock Suppressors at Quad-Citics Nuclear Power Station, Unit 1, AEC Docket 50-254, DHt-29

Dear Mr. O' Leary:

The purpose of this letter is to report the results of a Hydraulic Shock Suppressor inspection which was conducted on October 4,1974 on quad-Cities Unit 1.

Four (4) Parker-Hannafin, 33 Bergen-Patterson, and 14 Grinnell Shock Suppressors were inspected.

All Grinnell and Parker-Hannafin Suppressors contained sufficient fluid and appeared to be in acceptable condition. Ho fluid leakage was present with renpect to any of these suppressors.

Pergen-Patterson Shock Suppressors Sarial Numbered F-61284-1 (Tag 1 -21 ),

402594 (Tac 1-Ph), and 68290 (Tar; 1-13), uere found to contain insufficient quantities of hydraulic oil in their respective reservoirs. All other suppressors were observed to be satisfactory.

The three ch'ective Percen-Patterson Shock Suppressors were removed and disansembled.

Inspection revealed the cause of the leakage in one suppressor to be a small chip out of the rod packing. This chip, about one-sixteenth of nn inch, was believed to be caused in the installation of the packing, j

The cause of the necond suppressor fluid loss was determined to be a nut on an accumulator tie rod that was not torqued down properly. No apparent cause could be found for the third suppressor's lors of fluid.
