O'Reilly SPR 84-004 Page Two April 10, 1984 EVENT DESCRIPTION On March 30, 1984, at approximately 1430 hours0.0166 days <br />0.397 hours <br />0.00236 weeks <br />5.44115e-4 months <br />, it was identified by the Independent Safety Engineering Group that the fire dampers had not been visually inspected as required by Technical Specification Surveillance Requirement 4.7.10.1.b.
The 18 month surveillance was being tracked from a "Last Performed Date" of August 9, 1982, as a result of Fire Rated Assembly inspections conducted at that time.
Review of the documentation of these tests indicates that a visual inspection of the fire dampers was not performed.
Only the verification that the dampers were installed was documented.
Previously, the dampers were inspected as part of the Startup Program, February through May 1982.
Therefore, the dampers had not been visually inspected within the time frame as required by Technical Specification Surveillance Requirement 4.7.10.1.b.
PROBABLE CONSEQUENCE The consequence due to this event was the loss of assurance that the dampers would have functioned as required in the event of a fire.
During this period, the fire dampers were not challenged.
CORRECTIVE ACTION A 100% functional test of the fire dampers was initiated on March 31, 1984.
The program is being completed by a crew under the coordination of the Fire Protection Coordinator.
This action is estimated to be completed by May 15, 1984.
A one (1) hour roving firewatch of the entire plant was initiated by 1630 hours0.0189 days <br />0.453 hours <br />0.0027 weeks <br />6.20215e-4 months <br />, March 30, 1984.
The roving firewatch will remain in effect until satisfactory completion of the functional testing of fire dampers in each area or zone.
The fire dampers are currently listed in the Fire Barrier Surveillance Test Procedures (STPs) which were issued in January and February 1984.
Fire barrier inspections completed to date were incomplete in that the fire dampers had not been inspected.
Based on the Fire Protection Group's understanding of when the inspections were last performed, this was not of immediate concern.
The Fire Protection Coordinator and Maintenance personnel are developing a program whereby the fire damper inspections will be performed in conjunction with the annual u-
