~ Pc: Abnomal Occurrence 73-3, Ginna Station - Pailure of 1C Safety Injection Pump to start manually from Control Board on 6/11/73
Dear Mr. O'Reilly:
This morning ut. 0409, while the operntors ver perfoming a periodic test i,
(PT-2.1), tht. C Safety Injection Pump failed to start manually from liun 16 when the optrntor closed the control switch at. the control board.
The A 6 b Safety injection Pumps hsd been previously tested satisfactorily per PT-2.1 at 0350 and 0407. Unen the pump failed to start and the white di cagr eca.cn t 1inht illuminated on the breaker control switch, the operator innediately cleared the disagreement light and attempted a second start of 1
the C Safety Injection Pursp. This time it started ntisf actorily and PT-2.1 was successfully completed on the C Safety Injection Purop.
Af ter the test was completed, the Shif t Foreman had the operator start the C Safety Inject.fon Pump from Bus 14.
It started saticfactorily. The C S. I. Pu:Lp was then started successfully once again from Bus 16.
The electricians pelformed further investigation by using maintenance pro-cedure M-32, procedure for use of multi-cmp ter,t unit to check the overcur-rent trip devices. All devicco were tested and found to function satisfac-torily;'
The Plant Operations Review Comittee met and reco::rnended furtber tests be made of the control cwitch and breaker for the C Safety Injection pump feed Y.
