Dear Mr. O'Reilly:
This is to confirn c.y convercation with Mr. A. D.
Davis today to report Abnorn.a1 occurrence 74-9 at Ginna Station.
During the norning of May 30, 1974, while conducting the routine Monthly PT-9 Surveillance Test (Periodic Test - Undervoltage and Underfrequency Protectica 11A and lin - 4160 Volt Busses) one of the underfrequency relays

condition was being sinulated. This failure defeated one of the two channels of redundancy provided for underfrequency protection o,f this bus.
Inc faulty relay was replaced with a sparc relay. The spare relay vaa tested satisfactorily and declared operable at 1125 on May 30, 1974.
