Dear Mr. O'Reilly:

This is to confirm my phone call to Mr. Lee Spessard this morning to report AW-.1 nerovrance 74-14 at Ginna Station.

While conducting Periodic Test (PT-2.1), and following the satisfactory test of the 1-A an'd 1-3 Safety Injection Pomps, the 1-C Safety Injection' Pump which can be operated from either Bus 14 or Bus 16, failed to start manually on Bus 16 the first time it was tried.

It had started properly from Bus 14, but the first time it was tried from Bus 16, it failed to start. The second try was successful and the surveillance of the 1-C pump was completed. The 1-C pump was again started from Bus 14 and then twice more from Bus 16..

The failure of the 1-C Safety Injection Pump to start from Bus 16 has occurred

previously on the following dates: June 11, 1973 (Abnormal occurrence 73-3);

June 12, 1973 (Abnormal Occurrence 73-4); and April 6, 1974 (Abnormal Occurrence 74-4).

The PORC met this morning and recotr. ended a thor ~ough inspection.of the breaker by the Electricians. Any further action that is necessary will be carried out after the Electricians' report back to the PORC.
