Electro:natic relief valve 2-203--3C failed to operate while being tested at rated preocure.

CCNDITIC"3 PRIOR TO CCCU33 50P Unit-2 was operating at rated reactor pressure with the bypass valves open and the turbum off the systen. The unit was shutting down for a ucekend outage.

DESCRIPPION OP CCCURP.INCP At 0'!05 hourn cn June 13, 1975 electromatic relief valve 203-3c failed en operability tes',, exhibiting no bypass valve recrense and only a slicht temperature increase en the pilot valve recorder. Valve 20 L 33 had fai. led on .

May 26 (Report No. 50-237/75-32) during post-maintenance testing. Elect 9E$t3e relief valves A, D, and E were all tested cuecessiblly.

DESIGNATIC'I OF APPARE'JP CAUSE OF OCCURRF2:CE (Ccmponent Failure)

The failure of both electromatic valves 3C and 3B appeared to bo due to leaking ceal rings in the valve. This leakage caused a loss of pressure diffe"ential within the valves, clininating the notive force necessary for valve operation (see enclosed diagram). A Drconer Valve representative was preacnt during disassembly of the failed valves, nnd he concurred with the opinion that the worn noal rings were the probable cause of failure on both valven.

A!!AhYSIS OP O^CURRZCP Since the unit had been already scheduled for a weekend outage to repair electromatje relier valve 3B, the Technical Specifications were met for two inoperable electrematic relief valves; i.e. , the unit uculd be chut down dddn 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br />. The health and cafety of plant personnel and the public were not endangered by this occurrence.

C010HCTIVE A'TICN The 12:nediate corrective action was to check fuce continuity for valve 30. The circuit was determined to be operable. The unit continued a normal chutdown and cooldown to allow maintenance to begin.

Both electromaties were replaced uith rebuilt valves. Successful curveillancen on these valven were performed at 150 prig and at rated reactor preasure as the unit was broucht back up on June 16, 1975.

Dreaner Valve is currently evaluating a new type of ceal ring caterial to prevent recurrencea of this nature. Since the cause of failure for valve 33 has now been identified and corrected, thir letter will be submitted in licu of the follow-up report referred to in report no. 75-32.

FAILURE DATA Electromatic relief valve failurcs have occurred on Unit-3 January 16, 1974 (report no. 57-74) and on Unit-2 May 20 (report no. 75-30) and 15y 26 (report no. 75-32), 1975 The electrc~atic relief valves are 6" valves, catalogue no. 152 H o:, manufactured by Drescer Industrial Valve.
